La périnatalité représente une période de vulnérabilité majeure pour la mère, le fœtus et le nouveau-né, nécessitant une approche multidisciplinaire où l’anesthésie-réanimation-médecine du périopératoire (ARMPO) joue un rôle central. Face ces enjeux, la Société française d’anesthésie et de réanimation (SFAR) formule plusieurs recommandations organisationnelles. Elle préconise la présence d’au moins un anesthésiste-réanimateur sur site avec un recours opérationnel 24/7, et soutient les propositions de restructuration des réseaux et regroupement des plateaux techniques pour améliorer la sécurité des soins. La SFAR appelle également à une révision de la classification des maternités intégrant le niveau de soins critiques adultes, à la généralisation d’unités de soins intensifs obstétricaux et à la revalorisation financière de ces activités. La systématisation de la simulation in situ en équipe multidisciplinaire est enfin proposée comme levier essentiel de formation et de sécurité des soins.

The perinatal period represents a phase of major vulnerability for the mother, foetus, and newborn, requiring a multidisciplinary approach in which anaesthesia, intensive care, and perioperative medicine (ARMPO) play a central role. Faced with deteriorating perinatal mortality indicators in France, the French Society of Anaesthesia and Intensive Care (SFAR) sets forth several organisational recommendations. It advocates for the continuous on-site presence of at least one anesthesiologist-intensivist (24/7) with the possibility to be helped by another professional of anesthesiology based on a local organisation, along with the consolidation of obstetric units to reach critical activity thresholds. The SFAR also calls for a revision of the maternity unit classification to incorporate adult critical care capacity, the creation of dedicated obstetric intensive care units, and sustainable financial recognition of these activities. Finally, the systematic implementation of in situ multidisciplinary simulation training is proposed as a key lever for improving both professional competency and patient safety.
