Abstract

Artificial Intelligence (AI) holds unprecedented potential to transform the entire Alzheimer’s Disease and related dementias (ADRD) research and care continuum. During the 2025 Clinical Trials on Alzheimer’s Disease (CTAD) conference, the EU/US CTAD Task Force reviewed recent advances in the field, highlighting the emerging role of agentic AI, and discussed priorities to guide future AI-driven innovation in ADRD. Agentic AI co-scientists are emerging as transformative tools to accelerate scientific discovery by autonomously integrating multimodal data, generating and evaluating new hypotheses, and connecting previously siloed knowledge into actionable insights. Operationalized through end-to-end translational pipelines, such platforms can also link mechanistic insights to therapeutic design by automating molecular docking and drug repurposing. Complementary initiatives are creating digital wet labs for in silico experimentation. At the same time, agentic AI is increasingly being used to address pre-discovery bottlenecks by automating data harmonization and inferring missing information through digital twins and synthetic data. AI is also reshaping next-generation ADRD clinical trials, through automated assessment of clinical outcomes, precision participant stratification, individualized disease trajectory modeling and prediction of treatment response. These approaches could substantially improve trial efficiency through responder enrichment. Importantly, AI applications are expanding beyond discovery and clinical trials to support diagnosis, care navigation, and personalized disease management. To translate the momentum surrounding AI innovation into meaningful and sustainable impact across the ADRD continuum, the Task Force identified priorities including the development of AI-ready ecosystems, closer collaboration between clinicians, researchers and computer scientists, AI-enabled optimization of clinical trials and robust ethical and governance frameworks for responsible AI adoption.
