Les techniques neurodynamiques sont largement utilisées en kinésithérapie musculosquelettique évaluer et traiter les dysfonctions du système nerveux périphérique. Toutefois, la progression de la mise en tension neurale repose souvent sur un raisonnement clinique implicite et peu formalisé. L’objectif de ce travail est de formaliser le raisonnement clinique qui guide la mise en tension neurodynamique à travers la proposition d’un modèle conceptuel fondé sur les principes biomécaniques et physiologiques du tissu neural. Le modèle proposé s’appuie sur l’analyse de la réponse symptomatique du patient lors du testing neurodynamique afin de réguler dynamiquement la contrainte mécanique appliquée. La progression tensionnelle repose sur l’identification d’un seuil symptomatique initial et sur l’adaptation de la charge mécanique, en fonction de l’évolution immédiate des symptômes. Ce modèle peut être interprété comme un système adaptatif en boucle fermée dans lequel la réponse symptomatique agit comme indicateur indirect de la tolérance neurale.

Neurodynamic techniques are widely used in musculoskeletal physiotherapy for the assessment and management of peripheral nervous system disorders. However, the progression of neural tension during neurodynamic testing and treatment is often guided by implicit clinical reasoning that remains insufficiently formalized in the scientific literature. The aim of this work is to formalize the clinical reasoning underlying neurodynamic tension through the proposal of a conceptual model grounded in the biomechanical and physiological properties of peripheral nerve tissue. The proposed model relies on the patient's symptomatic response during neurodynamic testing to dynamically regulate the mechanical load applied to the neural structures. The progression of neural tension is guided by the identification of an initial symptomatic threshold (baseline), followed by controlled adjustments of mechanical stress according to the immediate evolution of symptoms. Within this framework, the model can be interpreted as an adaptive closed-loop regulatory system in which symptom responses act as an indirect indicator of neural tolerance to mechanical loading. This conceptual framework aims to structure clinical reasoning in the application of neurodynamic techniques and may provide a basis for future experimental investigations exploring the relationship between mechanical loading parameters and physiological neural responses.
