•	Overcrowding in emergency departments worldwide compromises care quality.

Abstract

Aim

The aims of this study were 1) to perform cross-cultural adaptation of the ED PREM into the French ED PREM; 2) to seek consensus on this version using the Delphi method and 3) establish its content validity.

Methods

This study was designed following the Consensus-based Standards for the selection of health Measurement Instruments guidelines. A cross-cultural adaptation of the ED PREM, renamed the French ED PREM, was performed. Consensus was sought through a Delphi method involving an expert committee, including the original developer and patient partners. Content validity was assessed with 50 patients and 7 healthcare professionals in the first round, and 10 patients in the second round, evaluating the relevance, comprehensibility, and comprehensiveness of the French ED PREM. The endpoint was an item-level content validity index (I-CVI) > 0.78 and a scale-level content validity index (S-CVI) > 0.90 without suggestions for revision.

Results

Consensus was achieved in two rounds, with 100% expert participation and agreement. All French ED PREM items achieved a robust content validity (I-CVI > 0.78, S-CVI > 0,90) and demonstrated excellent agreement among evaluators beyond chance (k > 0.74).

Conclusion

Through a rigorous cross-cultural adaptation of the ED PREM, this international collaborative research has demonstrated the content validity of the French ED PREM. This work contributes to the international harmonization of patient experience measures and may facilitate future comparisons across healthcare systems.
