The aim of this position paper is to propose a conceptual and clinical framework for conceptualizing severe and chronic illness as a specific psychopathological object, distinct from post-traumatic stress disorder. Research on trauma following severe illness has largely been built by transposing the post-traumatic stress disorder (PTSD) template: a past, external, bounded event whose sequelae are measured with dedicated scales. Edmondson’s Enduring Somatic Threat (EST) model exposed the limits of this transposition by showing that, in illness, the threat is internal, ongoing, and future-oriented. This paper extends that shift while relocating its theoretical ground. We argue that severe and chronic illness is not a somatic variant of PTSD but a specific psychopathological object, defined by an embodied threat that is partly real rather than merely perceived, an indefinitely open temporality, a techno-institutional mediation of experience, and the possible reactivation of prior adversity. Building on a process-based conception of adaptation, we contend that what is clinically at stake lies beyond coping and even beyond resilience: a forced transformation of the patient’s value system and relation to life and death. We defend three linked claims. First, the event-based paradigm and existing models, including EST, name this object without supplying its clinical framework or metapsychology. Second, the near-exclusive reliance on classical symptom measures risks reifying the object by capturing only its already measurable part. Third, case conceptualization articulating dual etiology and a temporal matrix is required before any protocol-driven application. From the nature of this object, we derive a clinical framework that does not claim to establish the efficacy of any technique, but defines the conceptualization requirements that any approach should meet before being evaluated in this field.
