Landiolol for the prevention of postoperative atrial fibrillation after cardiac surgery: a GRADE-assessed systematic review and meta-analysis of randomized controlled trials - 05/10/26

Background |
Postoperative atrial fibrillation (POAF) occurs in up to 30% of patients undergoing cardiac surgery. Landiolol is an ultrashort-acting, highly β1-selective intravenous beta blocker. We aimed to investigate the effect of landiolol in the prevention of POAF.
Methods |
Electronic databases were searched for randomized clinical trials (RCTs) evaluating landiolol for the prevention of POAF. Dichotomous outcomes were reported as odds ratios (ORs) and continuous outcomes as mean differences (MDs), each with 95% confidence intervals. The primary outcome was the incidence of POAF.
Results |
A total of 11 RCTs, including 1,264 patients, were analyzed. Landiolol was associated with a significantly lower incidence of POAF compared with the control group (OR: 0.35, 95% CI: 0.23−0.53, p < 0.01). There were no significant differences between landiolol and the control group in terms of hospital mortality, length of hospital stay, heart failure, or major complications.
Conclusions |
Landiolol appears to be an effective agent for the prevention of POAF in patients undergoing cardiac surgeries. Large-scale, multi-ethnic RCTs are needed to confirm these findings.
