Pour l’urologue en formation, l’enjeu est d’anticiper les décisions qui conditionnent la stadification et la suite du parcours : cystoscopie descriptive, cytologie, ECBU, traitements antithrombotiques, imagerie disponible, indication d’IRM vésicale pré-RTUV, prescription et logistique d’Hexvix, prélèvement urinaire pour biomarqueur ou registre TVNIM-AFU, stratégie de prélèvements anatomopathologiques et organisation du retour du résultat. Cette mise au point propose un parcours pratique centré sur trois piliers d’anticipation - IRM pré-RTUV, Hexvix et biomarqueur/registre – afin de sécuriser la qualité carcinologique de la première RTUV et les décisions après anatomopathologie.

Initial TURBT for bladder tumor is often considered mainly as an operative procedure, whereas a substantial part of its quality is determined before the patient enters the operating room. For urologists in training, the practical challenge is to anticipate the decisions that shape staging and the subsequent pathway: descriptive cystoscopy, urine cytology, urine culture, antithrombotic treatment, available imaging, indication for pre-TURBT bladder MRI, prescription and logistics of hexaminolevulinate, urine sampling for biomarker assessment or NMIBC registry inclusion, pathology-oriented sampling strategy and organization of pathology result review. This practical review focuses on three anticipatory pillars – pre-TURBT MRI, hexaminolevulinate and biomarker/registry workflow – to secure the oncological quality of the first TURBT and post-pathology decisions.
