How to prevent self-extubation in the intensive care unit?

Self-extubation is not an uncommon adverse event in mechanically ventilated patients in the intensive care unit. Although the incidence is relatively low when reported per ventilator-days, consequences may be significant when leading to urgent reintubation. It belongs to a broader category of unplanned extubations, that also includes accidental extubation, occurring during care, mobilization, or airway handling. The patient at risk is often in a “grey zone”: inadequately sedated, uncomfortable, confused or anxious, able to reach their endotracheal tube, but not yet extubated. Consequently, prevention cannot be limited to increasing sedation or using physical restraints. It relies on a multidimensional strategy: treating pain, dyspnea and discomfort, targeting protocolized light sedation, screening for delirium, avoiding delayed planned extubation, securing the endotracheal tube, and anticipating high-risk care procedures. More than half of self-extubated patients are not reintubated, reminding us that the best prevention is often timely planned extubation.
