Fifth and fourth metacarpal synostosis fusion without iliac grafting in management of fifth CMC osteoarthritis - 31/10/26

Abstract |
Fifth CMC osteoarthritis are not so rare. Among the available surgical treatment, the Dubert procedure has been proven efficient. It consists of resecting the base of the fifth metacarpal (M5) and creating a arthrodesis between M5 and the fourth metacarpal (M4). The results of this technique are satisfactory in several series. In its initial description, this technique used an iliac graft to create the synostosis and later proposed the use of the base of the fifth metacarpal as bone graft. We reviewed the outcomes of this technique when using the removed fragment of the 5 th metacarpal base only.
Four patients underwent surgery between 2018 and 2021. They were all men between 25 and 45 years old. The minimum follow-up was 20 months. Complete union was obtained in all four patients with a delay of 2.5 to 4.5 months. The pain was relieved. Postoperative wrist strength and mobility were identical to the contralateral side at last follow-up.
The Dubert procedure is a satisfactory procedure for relieving pain, regaining strength and partial mobility. However, it has been mostly described with the use of an iliac graft.
The modified technique used in our series appears to yield comparable results while avoiding iliac crest graft harvesting and its associated morbidity. It also reduces operative time and allows outpatient management under axillary block, thereby reducing costs.
