Abstract |
Background |
Quantification of peanut allergen immunoglobulin E (IgE) is essential for the diagnosis and monitoring of peanut allergy. While singleplex assay is widely used for component-resolved diagnostic, line blot assay (LBA) may allow simultaneous detection of multiple allergen-specific IgEs. However, analytical concordance between these methods has not been extensively evaluated.
Methods |
We conducted a retrospective multicentric study including 103 serum samples from children and adults with suspected peanut allergy. Specific IgE levels were measured using an ImmunoCAP platform (Ara h 1, 2, 3, 6, 8, and 9) and Euroline LBA (Ara h 1, 2, 3, 6, Bet v 1, and 9). Analytical agreement was assessed using Spearman correlation coefficients, Cohen’s kappa coefficients, and Bland–Altman analysis on log10-transformed data.
Results |
Strong quantitative correlations were observed across all components (r = 0.765–0.889, p < 0.001). Qualitative agreement was fair to moderate (κ = 0.290–0.506). Bland–Altman analyses demonstrated substantial individual variability between methods.
Conclusion |
Despite strong correlations, ImmunoCAP and Euroline showed limited qualitative agreement and wide limits of agreement, indicating that both methods are not directly interchangeable for quantitative peanut molecular IgE assessment. Nevertheless, Euroline may be useful for the broad characterization of component-specific IgE sensitization profiles, although its clinical utility remains to be established.
