Recovery of pre-symptomatic Tegner activity level after double-level osteotomy for medial compartment osteoarthritis: Outcomes at 24 months - 03/11/26

Abstract |
Background |
Double-level osteotomy (DLO) provides high return-to-sport rates, but these are generally based on the immediate preoperative activity level, which may already be reduced by knee pain. We evaluated recovery to a recalled pre-symptomatic activity level and associated factors.
Hypothesis |
Recovery would be less frequent against the pre-symptomatic reference than the immediate preoperative level.
Patients and methods |
This retrospective study included 51 DLO patients wishing to resume sports, with symptom durations below five years. Tegner activity scale (TAS) scores were recalled for five years before surgery and assessed immediately preoperatively and at 12 and 24 months. Exploratory Firth logistic regression included age, BMI, Kellgren–Lawrence grade, and preoperative Pain Catastrophizing Scale (PCS) to examine failure to recover the five-year TAS at 24 months.
Results |
Median [IQR] TAS was 5 [4–8] at the five-year reference, 3 [1–5] preoperatively, 4 [2–5] at 12 months, and 4 [3–7] at 24 months. At 24 months, 56.9% recovered the five-year TAS versus 88.2% recovering the preoperative TAS. Higher PCS was associated with failure to recover the five-year TAS (adjusted OR per point, 1.066; 95% CI, 1.008–1.143; p = 0.025).
Discussion |
Recovery proportions differed by reference, with interpretation limited by recall bias and patient selection. Higher PCS was associated with non-recovery in exploratory analysis.
Level of evidence |
III; retrospective cohort study.
