Unfavorable prognosis of pathogenic variants in the telomerase complex in cases of familial pulmonary fibrosis

Résumé |
Introduction |
Conclusion |

Introduction |
Pathogenic variants located in genes involved in telomere homeostasis (telomere-related genes [TRGs]) are associated with cases of familial pulmonary fibrosis (FPF). The aim of our study was to characterize the clinical presentation and prognosis of patients with FPF according to the presence or absence of a pathogenic variant involving a TRG.
Methods |
We conducted a retrospective, observational, single-center study including all patients with FPF followed at the Rare Lung Diseases Reference Center of Bichat–Claude-Bernard Hospital (Paris, France) and analyzed between 2021 and 2024, using next-generation sequencing of a single 30-gene panel associated with FPF.
Results |
Fifty-six patients were included. Median age at diagnosis of interstitial lung disease (ILD) was 65 years. Thirty-three patients (59%) exhibited a CT pattern of usual interstitial pneumonia (UIP), four (7%) had a nonspecific interstitial pneumonia (NSIP) pattern, and three (5%) displayed a bronchiolocentric interstitial pneumonia pattern. The diagnoses retained after multidisciplinary discussion (MDD) were as follows: idiopathic pulmonary fibrosis in 29 patients (52%), hypersensitivity pneumonitis in four (7%), ILD associated with rheumatoid arthritis in four (7%), and idiopathic NSIP in three (5%). A pathogenic TRG variant was detected in 12 patients (21%): TERT ( n = 4), PARN ( n = 4), RTEL1 ( n = 3), and TERC ( n = 1). The frequency of pulmonary and extrapulmonary manifestations at diagnosis did not differ significantly between patients with or without a pathogenic TRG variant. Patients carrying a pathogenic TRG variant underwent more rapid annual decline in forced vital capacity (−4.54 vs. −1.75 percentage points; P = 0.044).
Conclusion |
These results confirm the unfavorable prognostic impact of an identified pathogenic variant of a TRG in patients with FPF, which is associated with a particularly rapid decline in respiratory function.
