Introduction |
Discussion |
Conclusion |

Introduction |
Internalizing problems, which include anxiety and depressive disorders, are common in adolescence. These are often associated with emotional dysregulation, thus underscoring the importance of cognitive-behavioral therapy approaches that incorporate emotional regulation strategies.
Objective |
This study aimed to implement an emotion regulation-focused cognitive and behavioral therapy program for adolescents diagnosed with internalizing problems, and to assess its effects on anxiety and depressive symptoms, emotional regulation skills, and levels of alexithymia.
Patients and methods |
This experimental study involved 55 adolescents aged 12 to 17 years old, diagnosed with internalizing problems. Diagnosis was based on the Arabic short version of the Mini International Neuropsychiatric Interview for Children and Adolescents (MINI-KID). Participants were assigned to a therapy ( n = 30) or control group ( n = 25). Anxiety and depressive symptoms were assessed using the Child Depression Inventory (CDI) and the Hamilton Anxiety Rating Scale (HARS). Emotional regulation was evaluated using the Emotion Regulation Questionnaire for Children and Adolescents (ERQ-CA), and alexithymia using the Alexithymia Questionnaire for Children (AQC). Assessments were conducted before and after the intervention.
Results |
Among participants who completed the therapy ( n = 25), there was a statistically significant reduction in internalizing symptoms, as indicated by lower CDI and HARS scores ( P < 0.001). Significant improvements were also observed in emotional regulation skills (ERQ-CA) and alexithymia (AQC) ( P < 0.001). These changes were significantly greater than those observed in the control group. A positive correlation was found between anxiety levels (HARS scores) and emotional suppression (ERQ-CA subscale) ( P = 0.016; r = 0.475). Increased use of cognitive reappraisal was negatively correlated with alexithymia scores ( P = 0.047; r = −0.402).
Discussion |
The findings demonstrate that cognitive-behavioral therapy incorporating emotional regulation strategies is effective to reduce internalizing symptoms in adolescents. These results are consistent with theoretical models that emphasize the role of avoidance behaviors, cognitive distortions, and difficulties in emotional regulation in the development and maintenance of anxiety and depressive disorders.
Conclusion |
Cognitive and behavioral therapy with an emphasis on emotional regulation is a promising intervention for adolescents with internalizing problems. Enhancements in emotional regulation are associated with reductions in anxiety and depressive symptoms.
