Intérêt de l’éducation thérapeutique chez les malades souffrant de schizophrénie : le programme SCHIZ’EDUC - 24/02/15

Résumé |
Les prises en charge thérapeutiques des troubles schizophréniques incluent les médicaments psychotropes, en particulier antipsychotiques, et des interventions non médicamenteuses : éducation thérapeutique (ETP), psychothérapie, remédiation cognitive, réhabilitation psychosociale… L’ETP a fait l’objet de diverses recommandations nationales et internationales : Fédération française de psychiatrie (2003), American Psychiatric Association (2006), Société allemande de psychiatrie, psychothérapie et neurologie, National Institute for Health and Clinical Excellence (2009) et World Federation of Societies of Biological Psychiatry (2012). La méta-analyse Cochrane de Xia et al. (2011) recense, entre 1998 et 2009, 44 études contrôlées et randomisées représentant un total de 5142 participants. Les résultats sont favorables aux programmes d’ETP sur plusieurs paramètres : l’incidence de la mauvaise observance au traitement apparaît moins importante chez les patients ayant bénéficié d’un programme d’ETP ; le taux de rechute est, plus faible parmi les patients ayant bénéficié d’ETP, ce qui se traduit également par une diminution du risque de réhospitalisation chez ces patients ; le fonctionnement global des malades, leur qualité de vie et leur niveau de satisfaction vis-à-vis des soins. Ainsi, le traitement de quatre patients par ETP permet d’obtenir une amélioration clinique chez un patient supplémentaire, par rapport à des soins classiques. Littérature et recommandations insistent sur l’intérêt d’associer les familles des patients aux programmes d’ETP. L’intervention psychoéducative a comme objectifs d’aider le patient à reconnaître et accepter les limitations dues au retentissement fonctionnel de la maladie, de « déstigmatiser » la maladie, de guider le patient vers une attitude préventive (reconnaître les prodromes de la rechute) et de renforcer sa motivation à prendre un traitement. Nous présentons « SCHIZ’EDUC », un programme transversal, pluridisciplinaire, opérationnel d’ETP, récemment labellisé, qui s’adresse à toutes les personnes souffrant de schizophrénie en phase de stabilité de la maladie et à leurs familles. Il a été élaboré au sein du pôle de psychiatrie du CHU de Clermont-Ferrand avec l’appui de l’unité transversale ETP du CHU et avec la participation de l’association Unafam. Le programme propose trois modules selon les besoins des participants : connaître la maladie, gérer son traitement et développer son autonomie. Il utilise des supports variés et ludiques, et fait intervenir différents professionnels : infirmiers, psychiatres, addictologues, psychologues, pharmacien, diététicien, éducateur sportif, assistante sociale.

The therapeutic treatment of schizophrenic disorders includes psychotropic drugs, especially antipsychotics and non-drug interventions: Therapeutic Education (TE), psychotherapy, cognitive remediation, psychosocial rehabilitation. The TE has been the subject of various national and international recommendations: French Federation of Psychiatry (2003), American Psychiatric Association (2006), German Society of Psychiatry, Psychotherapy and Neurology, National Institute for Health and Clinical Excellence (2009) and World Federation of Societies of Biological Psychiatry (2012). Between 1998 and 2009, the COCHRANE meta-analysis by Xia et al. (2011) surveys 44 randomized and controlled studies, representing a total of 5142 participants. The results are favorable to TE programs on several parameters: the incidence of error observation to treatment appears to be less important in (for) patients following a TE program. The relapse rate was lower among patients who received TE, which also results in a decrease of the risk of patients’ rehospitalization, the general health status of patients, their life quality and their level of satisfaction towards the medical care. Thus, treating four patients, by using TE, provides a clinical improvement for an additional patient, compared to conventional care. Literature and recommendations emphasize the importance of involving patients’ families in TE programs. The psychoeducational intervention's objectives are to help the patient to recognize and accept the limitations imposed by the functional impact of the disease, to “destigmatize” the disease, to guide the patient towards a preventive approach (recognizing prodromal symptoms of relapsing) and to enhance the patient's motivation to follow a treatment. We present “SCHIZ’EDUC” a transversal, multidisciplinary and operational program of TE, recently certified, designed for people with schizophrenia in the stable phase of the disease and for their families. It was developed within the CHU Clermont-Ferrand Psychiatry Hospital with the support of the cross TE Hospital Department and with the participation of the Unafam Association. The program has three modules according to the needs of participants: knowing the disease, treatment guidelines and develop autonomy. It uses varied and entertaining media and it involves different professionals: nurses, psychiatrists, workers.
