Introduction

La rareté actuelle des manifestations de la syphilis tend à faire oublier et méconnaître certains aspects autrefois banals de l'affection. Nous rapportons 4 cas d'arthropathie tabétique colligés au service de Dermatologie du CHU Ibn Rochd de Casablanca.

Observations

L'âge moyen des malades était de 50 ans, tous de sexe masculin. Le diagnostic d'arthropathie tabétique était posé sur les critères cliniques, radiologiques et biologiques. Le contraste était frappant entre l'importance des déformations articulaires et l'indolence. L'aspect radiologique associait des lésions destructrices et constructrices. La sérologie syphilitique était positive dans le liquide articulaire, le sang et/ou le liquide céphalorachidien. Le traitement était essentiellement médical.

Discussion

L'arthropathie tabétique, affection exceptionnelle, est une des complications du tabès. Sa prévention repose sur le traitement de la syphilis à un stade précoce avant la survenue des formes articulaires et neurologiques, dont la prise en charge est difficile.

Tabetic arthropathy: “a forgotten disease”
Introduction

With the current sparcity of syphilitic manifestations, the once obvious aspects of the affection have been forgotten or are little known. We report 4 cases of tabetic arthropathy admitted to the Dermatology Department Ibn Rochd University Hospital of Casablanca.

Observations

The mean age of the patients was 50 years, all were males. The diagnosis was based on clinical, radiological, and biological criteria. There was a large contrast between the extent of the articular malformation and the indolence. The radiological aspect associated destructive and constructive lesions. The syphilis serology was positive in the articular liquid, the blood and/or the cerebro-spinal fluid. Treatment was essentially medical.

Discussion

Tabetic arthropathy, an exceptional affection, is one of the complications of the tabes dorsalis. Its prevention relies on the treatment of the syphilis at an early stage, before the onset of the articular and neurologic forms, the management of which remains very difficult.
