But de l’étude |
Le but de cette étude a été de déterminer la sensibilité aux antibiotiques de souches de Clostridium difficile isolées au cours de diarrhées nosocomiales.
Méthodes |
L’activité in vitro de 22 antibiotiques a été évaluée sur 401 souches de C. difficile isolées du 1er janvier 2001 au 31 août 2007 chez les patients hospitalisés au groupe hospitalier Chenevier–Mondor à Créteil. La sensibilité aux antibiotiques a été déterminée par la méthode des disques selon les recommandations du CA-SFM.
Résultats |
Toutes les souches étaient sensibles au métronidazole et à la vancomycine et toutes étaient résistantes à la clindamycine, à la gentamicine et à la colistine. Les souches étaient souvent sensibles à la pristinamycine (97,8 %), à la rifampicine (94,8 %) et au chloramphénicol (97,3 %) et elles étaient variablement sensibles à l’érythromycine et à la spiramycine (72,8 %) ainsi qu’à la tétracycline (85,6 %). À noter qu’aucune souche n’était sensible à l’ofloxacine et 80,5 % d’entre elles présentaient une résistance de haut niveau. Pour les β-lactamines, les souches étaient toutes résistantes aux céphalosporines testées et neuf souches (2,2 %) étaient résistantes aux pénicillines avec ou sans inhibiteurs de β-lactamases. Enfin, 28 (7 %) et 38 (9,5 %) souches étaient respectivement catégorisées intermédiaires et résistantes à l’imipénème avec un aspect de résistance hétérogène (colonies « squatters » dans la zone d’inhibition).
Conclusion |
Toutes les souches testées étaient bien sensibles au métronidazole et à la vancomycine qui représentent les deux antibiotiques utilisés dans le traitement des infections à C. difficile (ICD).

Aim |
The aim of this study was to determine the antimicrobial susceptibilities of Clostridium difficile clinical isolates obtained from symptomatic patients suffering from diarrhoea.
Methods |
In vitro activities of 22 antimicrobial agents were evaluated against 401 clinical isolates of C. difficile collected from patients hospitalized in a French university hospital (Henri Mondor hospital) between 2001 and 2007. The in vitro antibiotic susceptibility testing was performed using the disc diffusion method according to recommendations of the antibiogram committee of the French society for microbiology.
Results |
All strains were found to be susceptible to metronidazole and vancomycin but resistant to clindamycin, gentamicin, and colistin. Most of them were susceptible to pristinamycin (97.8%), rifampin (94.8%), and chloramphenicol (97.3%), and variable degrees of resistance were found for erythromycin and spiramycin (72.8%), as well as for tetracycline (85.6%). Note that none of the strain was susceptible to ofloxacin, and that 80.5% of them showed a high-level resistance. For β-lactams, all strains were resistant to tested cephalosporins, and nine isolates (2.2%) were also resistant to penicillins combined or not to a β-lactamase inhibitor. Finally, 28 (7%) and 38 (9.5%) of tested strains were categorized intermediate and resistant to imipenem (with a heterogeneous aspect), respectively.
Conclusion |
All tested strains were found to be susceptible to metronidazole and vancomycin which remain antimicrobial agents for the treatment of infections caused by C. difficile.
