Résumé

L’éclampsie du post-partum est une entité rare (11 à 44 % des éclampsies), pouvant parfois survenir tardivement, au-delà de la 48e heure. Nous rapportons un cas d’éclampsie du post-partum survenu au 12e jour après une grossesse et un accouchement non compliqués. L’éclampsie du post-partum diffère de l’éclampsie ante-partum par un tableau souvent pauci symptomatique ou inopiné. Le taux de mortalité est comparable à celui de l’éclampsie ante-partum. L’IRM paraît être un examen de choix en cas de doute diagnostique. Un traitement par sulfate de magnésium peut être envisagé selon les mêmes modalités qu’en ante-partum. Identifier et informer les patientes à haut risque permettrait un diagnostic précoce dont peut dépendre le pronostic de cette forme atypique d’éclampsie.

Abstract

Post-partum eclampsia is an unfrequent entity (11 to 44 % of eclampsia), which can sometimes occur late, beyond the 48th hour. We report a case of late post-partum eclampsia which occurred 12 days after an uncomplicated pregnancy and delivery. Post-partum eclampsia differs from antepartum by a pauci symptomatic or unexpected context. The mortality rate is comparable with that of the ante-partum eclampsia. Brain MRI appears to be the investigation of choice in case of uncertain diagnosis. Magnesium sulfate treatment should be considered under the same conditions as during pregnancy. Identifying and informing high-risk women would allow early diagnosis, which may improve the prognosis of this atypical type of eclampsia.
