Introduction |
Shewanella alga est un bacille Gram négatif commensal des eaux et des sols. Il est rarement impliqué en pathologie chez l'homme. Les atteintes sévères sont décrites chez des sujets immunodéprimés.
Observation |
Chez un homme de 66 ans, ayant une plaie du talon, survenait, après des bains de mer répétés dans l'Océan Atlantique (île d'Oléron, France), une dermohypodermite bactérienne aiguë de jambe avec septicémie à Shewanella alga. La porte d'entrée était l'ulcération talonnière au contact de l'eau de mer, responsable de l'infection malgré l'absence de déficit immunitaire. La guérison était obtenue après un traitement prolongé associant céfotaxime, ciprofloxacine et gentamicine intraveineuse, puis ciprofloxacine et érythromycine per os. Shewanella alga était aussi sensible à ticarcilline, piperacilline et ceftazidime, mais résistant à ampicilline, amoxicilline – acide clavulanique, colistine, cyclines, fosfomycine et cefsulodine. Aucun signe biologique de déficit immunitaire n'était identifié.
Discussion |
Il s'agit du premier cas à notre connaissance de dermohypodermite bactérienne aiguë à Shewanella alga avec septicémie, survenant chez un sujet immunocompétent. L'évolution a été favorable après antibiothérapie. Le traitement de cette dermohypodermite bactérienne doit être plus long que celui d'un érysipèle streptococcique, et l'antibiothérapie initiale ne doit pas se limiter à une aminopénicilline.

Background |
Shewanella alga is a Gram-negative bacilla often found in water or soil. Clinical infections in humans are rare, with serious infections described generally in immunocompromised hosts.
Case report |
A 66 year-old man with a heel wound had developed, after numerous sea baths in the Atlantic ocean (Oleron island, France), an infectious cellulitis of the leg with Shewanella alga septicemia. Despite the absence of immunodeficiency in this patient, infectious cellulitis and septicemia occurred via a wound to the skin and contact with sea water. Healing was seen after treatment with cefotaxime, ciprofoxacin and gentamicin IV, relayed with ciprofloxacin and erythromycin. Shewanella alga was also susceptible to ticarcillin, piperacillin and ceftazidime but was resistant to ampicillin, amoxicilline-clavulanate, colistin, cyclines, fosfomycin and cefsulodin. No immunological anomaly was found.
Discussion |
We report a case of infectious cellulitis with Shewanella alga septicemia. This has not previously been described in an immunocompetent patient.
Antibiotic therapy must be longer than usual treatment for streptococcal erysipelas and first-line antibiotherapy with ampicillin alone may not be sufficient.
