Résumé |
Le cancer gastrique est un cancer fréquent à l’échelle mondiale. La majorité des cancers gastriques sont sporadiques. Néanmoins, il existe 10 à 20 % d’agrégations familiales. Nous aborderons dans cette mise au point d’une part les formes non syndromiques de cancers gastriques d’origine héréditaire, notamment le cancer gastrique héréditaire de type diffus (CGHD), et les polyposes gastriques, dont la GAPPS (Gastric adenocarcinoma and proximal polyposis of the stomach), polypose proximale avec adénocarcinome gastrique, individualisée plus récemment, et d’autre part les formes syndromiques, notamment le syndrome HNPCC (Hereditary Non Polyposis Colorectal Cancer)/Lynch, qui surviennent dans le contexte d’un syndrome de prédisposition héréditaire aux cancers connus. Les progrès récents de l’oncogénétique et la généralisation des techniques de séquençage haut débit ont permis l’identification de nouvelles entités. Ainsi, la meilleure connaissance des syndromes de prédisposition au cancer gastrique héréditaire permet aujourd’hui une prise en charge plus précoce des patients et/ou de leur famille. Dans ce contexte, le pathologiste a un rôle essentiel dans la reconnaissance d’entités morphologiques spécifiques, qui va conduire à une enquête génétique. Ce chapitre a pour objectif de faire un état des connaissances sur les cancers gastriques héréditaires.

Gastric cancer is the third most common cancer worldwide. The majority of gastric cancers are sporadic but familial clustering is seen in more than 10% of cases. This manuscript is divided into two parts. The first part is dedicated to the non-syndromic hereditary gastric cancer, particularly the hereditary diffuse gastric cancer (HDGC) and other gastric polyposes including the recently described GAPPS (Gastric adenocarcinoma and proximal polyposis of the stomach). The second part concerns the syndromic gastric cancer, namely the HNPCC syndrome (Hereditary Non Polyposis Colorectal Cancer) occurring as part of a genetic predisposition syndrome to cancer. Recent advances in oncogenetics and next generation sequencing technology have enabled the identification of new entities. This enhancement in knowledge regarding inherited syndromes predisposing to gastric cancer has consequently improved the management of patients and their families. In this context, pathologists play a major role in identifying particular morphologic entities prompting genetic investigation. The aim of this manuscript is to provide an update on the current knowledge about hereditary gastric cancer.
