La surinfection et la transformation maligne sont des complications rares mais potentiellement sévères des endométriomes devant être reconnues par le radiologue. La suspicion de leur survenue rend nécessaire une exploration par un protocole d’IRM incluant des séquences injectées avec technique de soustraction. Certains signes sont communs aux deux entités, en particulier la modification des caractéristiques d’un endométriome connu (augmentation en taille, modifications de son signal typique). Des parois épaisses et fortement rehaussées, parfois associées à un aspect de pelvipéritonite, évoqueront une surinfection, tandis qu’une portion tissulaire rehaussée au sein de l’endométriome fera craindre une transformation maligne. Dans le cas ici décrit ces deux complications étaient associées.

Infection and malignant transformation are rare but potentially severe complications of endometriomas which must be recognized by radiologists. Suspicion of their occurrence must lead to investigations by MRI acquisitions which protocol includes injected series associated with subtraction images. Some elements are common to both entities, in particular modification of a known endometrioma characteristics (increase in size, loss of its typical signal features). Thick and strongly enhanced endometrioma walls, sometimes associated with pelvic peritonitis, are in favor of an infection, whereas an enhanced tissular component to the endometrioma points to a malignant transformation. In the case hereby described both complications were combined.
