The outcomes of stroke patients in health territory - 26/09/17

Objective |
The follow-up of stroke patients after the stroke center is a public health issue that motivate the implementation of the stroke plan and follow-up consultation. Its organization conduces us to evaluate patients flows from acute care to the rehabilitation ones.
Material/patients and methods |
We studied these flows from the hospital stays, according with the French system of valuation (PMSI) and from the transfers in rehabilitation centers, according with the French system of transfers (Via Trajectoire), to observe the follow-up of these stroke patients and to provide the organization of consultation. The data were obtained along the year 2015.
Results |
During 2015, 1408 stroke patients were treated in the stroke center of the Perpignan hospital. This rate represents 90% of the stroke patients treated on the health territory. In total, 71,3% were treated in the neurology unit, 27,7% in the neurosurgery or medicine ones. The death rate is 10%. It is finally 1270 stroke patients that enter in the care spectrum. In total, 50% of them return directly to home and 50% participate in a rehabilitation program (30% in a specialized program, 20% in a polyvalent one).
Discussion – conclusion |
This study conduces us developing a repertoire of stroke patients and sharing follow-up consultations between the different operators on the heath territory. It is extended to the neighboring territory, around the Narbonne hospital, linked with the Perpignan one, into a hospital association (GHT).
