Objectives |
To report long-term outcomes after I-Stop TOMS™ implantation for PPI.
Patients and methods |
A retrospective evaluation was conducted in three tertiary reference centers. All consecutive patients implanted with an I-Stop TOMS™ sling between 2007 and 2012 for mild to moderate PPI (24-hour Pad test<400g) without history of pelvic radiation therapy were included. Evaluation had been conducted preoperatively, at one and six months postoperative and yearly thereafter. The main outcome criterion was the number of pads per day. Secondary criteria were International Consultation on Incontinence Questionnaire (ICIQ), SF-36 questionnaire, and complications.
Results |
A hundred patients were evaluated with a median follow-up of 58months [19–78]. Pad use was significantly reduced and quality of life improved at last follow-up (P<0.0001). The percentage of patients dry and socially continent (0 or 1 pad) were 40% and 77% at 1 year, then dropped to 15% and 22%, respectively after 5years. Twelve patients were treated by artificial urinary sphincter implantation, five by ProACT™ balloons and one by a re-do I-Stop TOMS™. No severe complications were recorded at last follow-up.
Conclusions |
I-Stop TOMS™ implantation is a safe and effective option in the short-term for mild to moderate PPI management. However, a significant trend to recurrence of leakage has been established after long-term follow-up. If confirmed by further studies, these results may substantially impact patient information before male sling implantation.

Rapporter les résultats à long terme de l’implantation d’une bandelette I-Stop TOMS pour le traitement de l’incontinence urinaire post-prostatectomie.
Méthodes |
Une étude rétrospective a été menée dans trois centres de référence. Tous les patients consécutifs traités avec une bandelette I-Stop TOMS entre 2007 et 2012 pour une incontinence urinaire post-prostatectomie légère à modérée sans antécédent d’irradiation pelvienne ont été inclus. L’évaluation a été réalisée en préopératoire, puis à 1 mois, 3 mois, 6 mois et annuellement ensuite. Le critère principal étudié était le nombre de protections par jour. Les critères secondaires d’évaluation étaient les questionnaires International Consultation on Incontinence Questionnaire (ICIQ) et SF-36 ainsi que les complications.
Résultats |
Cent patients ont été évalués avec un suivi médian de 58 mois [19–78]. Le port de protections était significativement réduit et la qualité de vie améliorée au dernier suivi (p<0,0001). Le pourcentage de patients secs ou socialement continents (0 ou 1 protection par jour) était de 40 % et 77 % à 1 an, puis a chuté à respectivement 15 % et 22 % après 5 ans. Douze patients ont été traités par sphincter artificiel, 5 par ballonnets et un par une seconde bandelette I-Stop. Aucune complication sévère n’a été notée au cours du suivi.
Conclusions |
La bandelette I-Stop TOMS™ est un dispositif sûr et efficace à court terme pour les incontinences post-prostatectomie légères à modérée. Cependant, une nette tendance à la récidive des symptômes a été constatée au cours du suivi. Si ces données sont confirmées par d’autres, elles pourraient influencer l’information du patient en préopératoire.
