Résumé |
L’hyperpigmentation post-inflammatoire (HPPI) se définit par l’apparition d’une hyperpigmentation cutanée survenant après et parfois pendant un processus inflammatoire. Bien que plus fréquente chez les personnes de phototype élevé, elle s’observe sur tout type de peau et à tout âge. Le retentissement sur la qualité de vie des personnes touchées est souvent majeur. La physiopathologie de l’HPPI reste en grande partie méconnue. L’activation de la stimulation mélanocytaire s’observe dès la 1re semaine soulignant l’importance de mesures préventives précoces. La photoprotection avec un écran offrant une protection équilibrée en ultraviolet (UV) B et A est indispensable. La lumière visible pourrait également jouer un rôle dans les HPPI mais son impact réel doit encore être étudié. En post-acte, des topiques cicatrisants avec activités anti-inflammatoires sont conseillés. Lorsque le risque d’HPPI est fort ou en cas de survenue d’HPPI, les dermocorticoïdes restent le traitement de choix.

Post-inflammatory hyperpigmentation (PIH) is a hyperpigmentation of the skin occurring after and sometimes during an inflammatory process. Although more frequent in dark skinned individuals, PIH can be observed in any type of skin and at all ages. In most case a strong impact on the quality of life of affected individuals is observed. The pathophysiology of PIH remains largely unknown. The activation of the melanocytes occurs in the first week following the inflammation emphasizing the crucial role of early preventive measures. Photoprotection with balanced UVA and UVB protection is required. Visible light could also play a role in PIH but this remains to be demonstrated. Healing topics with anti-inflammatory properties are of interest after a skin procedure. When the risk of PIH is high or when PIH occurs, topical steroids remains the gold standard approach.
