Critères diagnostiques en échographie-Doppler des dissections artérielles cervicales à la phase aiguë - 19/03/08

Voir les affiliationsObjectif. Évaluer la valeur de l’écho-Doppler dans le diagnostic initial des dissections artérielles cervicales.
Matériel et méthode. Étude rétrospective de 31 patients d’âge moyen 45 ans, adressés pour suspicion de dissection artérielle cervicale confirmée en IRM qui a diagnostiqué 46 artères disséquées (35 carotides internes et 11 vertébrales) chez 31 patients. Dix patients présentaient des dissections multiples. Les critères diagnostiques évalués en écho-Doppler étaient les signes échographiques directs d’hématome pariétal : élargissement du calibre externe de l’artère, réduction de la lumière circulante, hématome hypo et/ou isoéchogène, lambeau intimal. Les critères associés recherchés étaient la topographie de la lésion et en présence d’une occlusion d’un axe, la dissection associée d’un autre.
Résultats. En écho-Doppler, on distinguait les segments artériels visualisables en échographie (carotide interne sus-bulbaire, vertébrale de V0 à V3) des segments non visualisables pour lesquels seul un enregistrement Doppler pulsé était possible (carotide intra-pétreuse, vertébrale en V3V4). Avec les critères retenus, le diagnostic de dissection était porté sur l’écho-Doppler initial dans 83 % des cas sur les segments visualisables et dans 30 % des segments non visualisables.
Conclusion. Les signes directs d’hématome pariétal sur les segments artériels visualisables en échographie ont une bonne sensibilité. Le choix de confronter l’image de l’hématome pariétal en écho-Doppler et en IRM limitait l’analyse des résultats à la seule sensibilité sans appréciation de la spécificité des critères retenus. La présence de dissections multiples doit être systématiquement recherchée puisque rencontrée dans 30 % des cas de notre série.

Purpose. To estimate the value of duplex color-coded ultrasonography in the initial diagnosis of acute cervical artery dissection.
Material and method. Retrospective study of 31 patients, mean age 45, sex ratio =1, referred for clinical suspicion of cervical artery dissection, confirmed by MRI. 46 dissected arteries were imaged. Ten patients presented multiple dissections. The evaluated sonographic diagnostic criteria were the direct signs of intra-mural hematoma: localized increased diameter of the artery, narrowed lumen, hypo and/or isoechoic intra-mural hematoma, intimal flap. The associated criteria studied were: the location of intra-mural hematoma and in case of an occlusion, the dissection of an other artery.
Results. We analyzed separately the arterial segments visualized in B-mode ultrasound (supra-bulbar internal carotid artery, vertebral artery from V0 to V3) and the arterial segments evaluated only by pulsed Doppler (intrapetrosal carotid artery, V3-V4 of the vertebral artery). With the above criteria, on arterial segments visualized in B-mode ultrasound, the diagnosis of dissection was done in 83% of cases during the initial examination but in only 30% of the arterial segments non visualized on ultrasonography.
Conclusion. The direct signs of intra-mural hematoma on internal carotid and vertebral segments imaged by B-mode ultrasonography have a good sensitivity. Because results were compared to MRI, the specificity could not be assessed. In case of occlusion of an artery, multiple dissections are a strong argument for the diagnosis.
