Decompressive Craniectomy and CSF disorders in children - 19/07/18

Introduction |
Even if decompressive craniectomy is a lifesaving procedure, it may be associated to several postoperative complications, namely cerebrospinal fluid (CSF) dynamics impairment.
Material and methods |
We performed a retrospective multicentric study analysis of CSF disorders in children aged from 0 to 17 years undergoing a DC during the hospitalization for TBI between January 2006 and August 2016 in the major departments of pediatric neurosurgery of France.
Results |
Hundred and fifty children underwent a DC for TBI between January 2006 and August 2016. The incidence of CSF disorders was 10, 67%. One main factor appeared statistically related to shunt placement: previous EVD placement. Shunt placement increased the risk of further complications especially cranioplasty infection (P=0.008).
Conclusion |
CSF disorders may develop several months after DC and its occurrence increases significantly the risk of complications of the cranioplasty. Clinical and radiological close follow-up is mandatory after discharge from the neurosurgical unit.
