Résumé |
Introduction |
Cette étude longitudinale prospective réalisée au sein de l’Unité de réadaptation fonctionnelle en milieu professionnel du Patis-Fraux (Vern-sur-Seiche, Ille-et-Vilaine) avait pour objectif de déterminer le taux de patients en emploi ou en formation à 18 mois de la sortie de l’unité. L’objectif secondaire était d’identifier des facteurs associés à un sur-risque de désinsertion professionnelle.
Méthodes |
L’inclusion était systématiquement proposée aux patients majeurs intégrant le programme entre le 1er février et le 31 octobre 2015. La sortie prématurée d’hospitalisation avant la fin de la quatrième semaine était un critère d’exclusion. Les patients étaient recontactés par téléphone à 6, 12 et 18 mois. Le critère de jugement principal était le taux de patients en emploi ou en formation à 18 mois de la fin du programme.
Résultats |
Cent vingt-deux patients ont été inclus. Cinq patients sortis précocement ont été exclus en cours d’étude. Le taux de retour en emploi à 18 mois était de 61,1 % (42,2 % en emploi et 18,9 % en formation). Le maintien dans l’entreprise restait minoritaire et les salariés retrouvaient essentiellement un emploi en dehors de leur entreprise initiale : il s’agissait alors surtout de réorientation professionnelle. La durée d’arrêt de travail et le fait de ne pas se sentir capable de reprendre un travail étaient associés à un sur-risque de désinsertion. En analyse multivariée, l’âge supérieur à 30 ans, l’arrêt de travail supérieur à 12 mois, le fait d’être adressé par un médecin du travail plutôt qu’un médecin généraliste, et l’IMC supérieur ou égal à 25kg/m2 étaient identifiés comme facteurs prédictifs défavorables.
Conclusion |
Le retour en emploi après ce programme est encourageant. Les facteurs prédictifs défavorables doivent être connus comme des signaux d’alertes par les professionnels de santé.

Introduction |
This prospective longitudinal study carried out within the Functional Rehabilitation Unit in Professional Environment of Patis-Fraux (Vern-sur-Seiche, Ille-et-Vilaine) aimed to determine the rate of patients in employment or training at 18 months of the exit of the unit. The secondary objective was to identify factors associated with an occupational disinsertion.
Methods |
Inclusion was systematically offered to major patients integrating the program between February 1 and October 31, 2015. The premature discharge of hospitalization before the end of the fourth week was an exclusion criterion. Patients were contacted by telephone at 6, 12 and 18 months. The outcome was the rate of patients in employment or training at 18 months from the end of the program.
Results |
One hundred and twenty-two patients were included. Five patients who were discharged early were excluded during the study. The reintegration rate at 18 months was 61.1% (42.2% in employment and 18.9% in training). The maintenance in the company was minority and employees found essentially a job outside their initial company: it was mainly about career reorientation. The duration of work stoppage and the fact of not being able to return to work were associated with an overabundance of disinsertion. In multivariate analysis, age greater than 30, work stoppage greater than 12 months, being addressed by an occupational physician rather than a general practitioner, and BMI greater than or equal to 25kg/m2 were identified as unfavorable predictors.
Conclusion |
Returning to work after this program is encouraging. Adverse predictive factors must be known as warning signals by the health professionals.
