General scenario of osteoarthritis

À ce jour, nous ne disposons d’aucun traitement spécifique de l’arthrose. La difficulté à trouver une thérapeutique efficace à la fois sur le plan symptomatique et structural tient d’abord à l’important retard au diagnostic lié au caractère longtemps asymptomatique de cette pathologie et à une définition largement radiographique, n’identifiant donc pas les stades les plus précoces. Cela est également lié à la complexité physiopathologique de la maladie (ou syndrome) arthrosique, impliquant l’ensemble des tissus de l’articulation, un dialogue cellulaire complexe et de nombreux médiateurs inflammatoires et cataboliques. Les mécanismes pathogéniques initiateurs, souvent dépassés au moment du diagnostic, dépendent au moins en partie du phénotype auquel se rattache l’arthrose du patient. Quatre principaux phénotypes sont maintenant identifiés : l’arthrose métabolique, génétique, traumatique et liée au vieillissement. Après avoir dessiné succinctement les contours épidémiologiques et sémiologiques de la pathologie arthrosique, nous allons tenter au cours de cette mise au point d’en résumer les principaux mécanismes physiopathologiques.

To date, there is no specific treatment for osteoarthritis (OA). The difficulty in developing a therapy that is effective both symptomatically and structurally is due first to the significant delay in diagnosis linked to the long asymptomatic period and to a largely radiographic definition, therefore not identifying the earliest stages. But this is also linked to the pathophysiological complexity of OA disease (or syndrome), involving all joint tissues, a complex cellular dialogue and many inflammatory and catabolic mediators. The initiating pathogenic mechanisms, often overcome at the time of diagnosis, depend at least in part on the OA phenotype. Four main phenotypes are now identified: metabolic, genetic, traumatic and age-related OA. After drawing the epidemiological and semiological outlines of OA pathology, we will try in this review to summarize the main pathophysiological mechanisms.
