Les patientes porteuses d’un variant délétère peuvent décider d’entreprendre une chirurgie de réduction des risques (CRR) afin de se soustraire à une surveillance clinico-radiologique lourde et anxiogène. Cette chirurgie prometteuse et néanmoins hautement mutilante. Notre objectif était d’évaluer la satisfaction post-opératoire chez ces patientes indemnes de cancer, afin d’améliorer nos pratiques.

Étude descriptive transversale, unicentrique. Notre critère de jugement principal est l’évaluation de la satisfaction post-opératoire à l’aide du questionnaire BREAST-Q. Pour interpréter ces résultats nous les avons comparés à ceux issu d’une cohorte de patiente indemne de toute chirurgie ou cancer du sein. Un test de Student pour échantillon unique a été utilisé. Sur une analyse secondaire nous avons étudié les facteurs influençant la satisfaction post-opératoire.

Les moyennes obtenues aux différents questionnaires sont de 56,06/100 pour « Satisfaction with breasts » ; 66,94/100 pour « Psychosocial weel-being » ; 50/100 pour « Sexual well-being » et 59,22/100 pour « Satisfaction with information ». La satisfaction postopératoire est comparable au groupe contrôle concernant le questionnaire « Satisfaction with breasts » (p=0,37) et « Psychosocial weel-being » (p=0,18). Concernant le questionnaire « Sexual well-bieng » il existe une différence entre nos groupe (p=0,01).

La satisfaction post-opératoire de nos patientes opérées semble superposable à celle d’une population n’ayant pas eu d’intervention mammaire sauf sur la question de la qualité de vie sexuelle qui semble impactée. Les analyses secondaires de notre étude nous ont également permis de mettre en lumière l’importance de l’information préopératoire pour le bien-être postopératoire.

Patients with very high risk of cancer mutation may decide to undergo prophylactic surgery in order to avoid heavy clinical and radiological monitoring. This is a promising and nonetheless risky surgery because it is a complicated procedure and highly mutilating. Our goal was to improve the practice of this prophylactic surgery, to do so we assessed a postoperative satisfaction survey to cancer-free patients who have undergone this procedure.

Single-center, cross-sectional descriptive study. Assessment of the primary efficacy endpoint using the BREAST-Q questionnaire. These results were compared to a control group representative of our sample of patients, those data came from the scientific literature using a single-sample Student's test. Several multivariate analyzes were also carried out in order to study the influence of certain factors on the patient's satisfaction.

The averages obtained in the various questionnaires were 56.06/100 for “Satisfaction with breasts”; 66.94/100 for “Psychosocial well-being”; 50/100 for “Sexual well-being” and 59.22/100 for “Satisfaction with information”. Postoperative satisfaction in our sample is comparable to the group control concerning the questionnaire “Satisfaction with breasts” (P=0.37) and “Psychosocial well-being” (P=0.18). Concerning the questionnaire “Sexual well-being” there is a significant statistical difference between our group and the control group (P=0.01).

The post-operative satisfaction of our operated patients seems to be proportionally similar to the general population who have not undergone breast surgery except on the question of the quality of sexual life. The various analyzes of our study also allowed us to highlight the importance of preoperative information for postoperative well-being.
