Cet article se propose de mettre en évidence de quelle façon un dispositif psychothérapeutique de groupe en institution constitue un objet d’élaboration en amont de l’expérience clinique à proprement parler. Notre hypothèse est que la dimension psychothérapeutique et le pouvoir dynamique d’un groupe sont déterminés par la conception que le clinicien se fait de son dispositif.

À l’appui de plusieurs situations cliniques de dispositifs institutionnels créés ou repris, issus de l’expérience des auteurs, nous avons distingué différents champs ou objets d’analyse. Ces objets sont en position d’interface mutuelle et font du dispositif une création propre au clinicien.

Différents rapports peuvent ainsi constituer des obstacles internes à la création et la mise en œuvre du dispositif. On distinguera le rapport du clinicien à son équipe et son institution, celui du dispositif au cadre institutionnel, celui du dispositif aux différents groupes auxquels le clinicien appartient, celui du clinicien à sa clinique.

Les exemples proposés montrent ici comment, selon des conditions d’agencement spécifique entre le clinicien, ses appartenances professionnelles, son équipe et la situation institutionnelle, quel point plutôt qu’un autre apparaît comme un élément de souffrance, de confusion ou de conflictualité à élaborer, discriminer ou dénouer.

Les éléments qui composent la conception que le clinicien a de son dispositif ne sont pas à explorer a priori, mais constituent potentiellement des objets d’élaboration qui apparaissent en amont ou dans les premiers temps de l’expérience clinique.

This article highlights how the creation of a group psychotherapeutic setting in an institution constitutes a subject of elaboration before the clinical experience itself. Our hypothesis is that the psychotherapeutic dimension and the dynamic power of a group are determined by the conception that clinicians have of their setting.

Using the examples of several clinical situations (in an institutional setting) that we either created or inherited, we have distinguished different fields or objects of analysis. These objects are in mutual interface and make the therapeutic project a creation of the clinician.

Different relationships can thus constitute internal obstacles to the creation and implementation of different types of therapy. We will distinguish: – the relationship of clinicians to their team and to their institution; – the relationship of the therapeutic project to the institutional framework; – the relationship of the therapeutic project to the different groups to which clinicians belong; – that of clinicians to their customary field of practice.

The examples proposed here show how, according to the conditions of specific arrangements between clinicians, their professional affiliations, their team and the institutional situation, different points may appear as elements of suffering, confusion, or conflicts to be elaborated, discriminated, or resolved.

The elements that make up the clinician's conception of their therapeutic initiative do not necessarily need to be explored before the project's implementation but potentially constitute objects of elaboration that might appear before or in the early stages of the clinical experience.
