L’acanthome du lit est une tumeur épithéliale bénigne de la matrice distale et du lit, proche des cas rapportés sous les termes de kératose sous-unguéale, kératose séborrhéique ou matricome onychocytaire. Il se manifeste comme une mélanonychie longitudinale associée à un épaississement de la tablette ou une leucoxanthonychie, bien plus rarement comme une érythromélanonychie ou une érythroxantholeuconychie. Nous pensons que ces lésions appartiennent toutes au même spectre. Le terme d’acanthome longitudinal nous paraît le mieux adapté car il décrit un processus clinico-pathologique. L’aspect clinique variable des lésions est sans doute lié à leur site anatomique exact au sein de l’épithélium de l’appareil unguéal.

Nail unit longitudinal acanthoma is a benign epithelial proliferation of the distal matrix and nail bed quite similar to other cases reported as longitudinal keratosis, seborrheic keratosis or onychocytic matricoma. Certain of the lesions are characterized by a longitudinal streak with localized nail-plate thickening while others present as leukoxanthonychia and nail thickening. Erythromelanonychia or erythroleukoxanthonychia are possible but very rare. We consider all these tumors to belong to a single spectrum of diseases. To avoid confusion, we propose to call them nail unit longitudinal acanthoma, which describes a histopathological process. The distinct clinical features of these tumors, leukoxanthonychia or melanonychia may be linked to their variable anatomic locations in different zones of the nail unit.
