Pitfalls in the interpretation of microbiological controls of the environment |
There is a general agreement for patients, health care workers and visitors to expect a high level of sanitation in our medical facilities. Nevertheless the hospital environment is naturally contaminated by microorganisms. Their contribution in the acquisition and spread of endemic nosocomial infections is poorly documented, except in case of outbreaks (invasive aspergillosis, nosocomial legionellosis). The control of environmental contamination relies on the application of basic rules of hygiene (preventive maintenance of the ventilation systems and water treatment systems, quality of cleanup procedures,…). The routine monitoring of environment is of limited value.
Controls should be selective and performed only after the analysis of the critical control points adapted to each situation. They must be carried out according to a standardized methodology. Results must always be interpretated according to the goals and to different levels of acceptability defined locally or defined by scientific agencies.
