Ces recommandations sont un consensus d’experts français glaucome et rétine sur la gestion de l’hypertonie oculaire observée dans 1/3 des cas après injection d’un implant de corticoïde. Elles réactualisent les premières recommandations éditées en 2017. Deux implants sont sur le marché français : l’implant de dexaméthasone (DEXi) et de fluocinolone acétonide (FAci). Il est important de connaître le statut pressionnel avant d’injecter un patient par implant de corticoïdes. La surveillance de la PIO adaptée à la molécule est nécessaire tout au long du suivi et des réinjections. Les études de vraie vie ont permis d’optimiser l’algorithme de prise en charge en augmentant significativement la sécurité d’emploi de ces implants. Un test corticoïde par DEXi est nécessaire avant un switch vers FAci pour optimiser la tolérance pressionnelle de ce dernier. Au-delà des traitements topiques hypotonisants, le laser SLT peut être considéré dans l’arsenal thérapeutique pour la gestion de l’HTO cortisonique et des injections futures.

These are the recommendations of French glaucoma and retina experts on the management of ocular hypertension (OHT) observed in 1/3 of cases after intravitreal steroid implant injections. They are an update to the recommendations first published in 2017. There are two implants on the French market: the dexamethasone (DEXi) and fluocinolone acetonide (FAci) implants. It is important to know the pressure status before injecting a patient with a steroid implant. Monitoring of the IOP adapted to the specific drug is necessary throughout follow-up and reinjections. Real-life studies have made it possible to optimize the management algorithm by significantly increasing the safety of use of these implants. A corticosteroid test with DEXi is necessary before switching to FAci to optimize the pressure tolerance of the latter. In addition to topical glaucoma medications, SLT laser can be considered in the therapeutic arsenal for the management of steroid-induced OHT and future injections.
