Decompressive craniectomy versus craniotomy for traumatic acute subdural hematoma - 14/09/23

• | Hutchinson trial compared the efficacy of decompressive craniectomy and craniotomy for the surgical treatment of traumatic acute subdural hematoma. |
• | Randomization was performed intraoperatively, once the bone flap was elevated. |
• | Patients whose brain was to swollen for the bone flap to be safely replaced were excluded from the trial. |
• | No difference was found in the neurological outcome between the decompressive craniectomy group and the craniotomy group. |
• | Patients who were excluded because of intraoperative brain swelling were the one who really required decompressive craniectomy, which induces a selection bias and thus precludes from generalizing the results to all the patients operated on for acute subdural hematoma. |
