The authors of the article “Correlation of a distended urinary bladder volume with lethal intoxication in general and for different substance subgroups in particular: A retrospective study” (Toxicologie Analytique et Clinique, 2023) examined the relationship between urine bladder volume and several substance groupings and assessed the repeatability of a urinary bladder volume cutoff of >330mL as a marker of fatal intoxication. It was discovered that bladder volume had good discriminatory performance for opioids, cocaine, benzodiazepines and amphetamines as the leading chemicals, but not for psychiatric medications and z-drugs, as well as for deadly intoxication in general. We applaud the authors for their fascinating and pertinent study, however we would like to bring out a few methodological and interpretive concerns that could compromise the reliability and applicability of their findings. We also propose some lines of inquiry for this area of study in the future.
