L’endophtalmie postopératoire est secondaire à une infection bactériologique et survient le plus souvent dans un mode aigu, quelques jours après la chirurgie. Toute inflammation postopératoire doit faire évoquer une endophtalmie aiguë, a fortiori s’il existe une inflammation vitréenne. La suspicion diagnostique doit s’associer à une prise en charge thérapeutique urgente afin d’améliorer le pronostic anatomique et fonctionnel. Les diagnostics différentiels sont le syndrome toxique, l’uvéite phaco-antigénique et l’inflammation d’origine mécanique liée à une malposition de l’implant. Chaque entité nécessite un traitement spécifique.

Postoperative endophthalmitis is a bacterial infection that most often occurs acutely a few days after surgery. Any postoperative inflammation should raise the suspicion of acute endophthalmitis, especially if vitritis is present. Suspicion of the diagnosis must be combined with emergent therapeutic management in order to improve the anatomical and functional prognosis. The differential diagnoses are toxic anterior/posterior segment syndrome, phacoantigenic uveitis and inflammation of mechanical origin linked to implant malposition. Each entity requires its own specific treatment.
