Simultaneous transcatheter aortic valve replacement and liver transplantation - 02/04/26

Managing patients with concomitant severe aortic stenosis (AS) and cirrhosis presents significant clinical challenges due to the complex interplay between cardiac and hepatic dysfunction. Emerging evidence suggests that transcatheter aortic valve replacement (TAVR) may be a safe and effective intervention in cirrhotic patients with severe AS, potentially serving as a bridge to restore transplant candidacy. We report two cases of decompensated cirrhotic patients who successfully underwent simultaneous TAVR and liver transplantation (LT). These cases illustrate the feasibility of combined cardiac and hepatic procedures in selected high-risk patients. However, such interventions require meticulous preoperative evaluation, multidisciplinary coordination, and institutional expertise.
