Nous avons réalisé une étude rétrospective intéressant 39 cas de lymphomes cérébraux primitifs (LCP) colligés dans notre service d’hématologie sur une période de treize ans allant de janvier 2011 à décembre 2023. À travers ce travail, nous visons à dresser un état des lieux concernant les LCP chez nos patients. L’âge moyen était de 48 ans, avec une prédominance masculine. La symptomatologie clinique était polymorphe et peu spécifique, mais dominée par les déficits neurologiques en foyer. Les lésions étaient habituellement uniques et se localisent à l’étage sus-tentoriel. Le diagnostic anatomopathologique était obtenu après biopsie stéréotaxique chez 32 patients, et après exérèse chirurgicale chez sept patients. Le type histologique le plus fréquemment retrouvé était le lymphome diffus à grandes cellules B. Tous les patients ont été inclus dans le protocole LCP : polychimiothérapie à base de méthotrexate haute dose puis consolidation par radiothérapie encéphalique. L’évolution des patients s’est caractérisée par la survenue d’une rechute chez dix patients dont huit en cérébral et deux en testiculaire. Au total, l’évaluation du protocole LCP a montré : une rémission complète chez quatorze patients, un décès chez seize patients et neuf patients ont été perdus de vue. La survie globale et sans progression à cinq ans était de 40,36 %, et de 36,7 % respectivement.

We conducted a retrospective study of 39 cases of primary cerebral lymphoma (PCL) collected in our hematology department over a 13-year period from January 2011 to December 2023. Our aim was to provide an overview of PCL in our patients. The mean age was 48 years with a male predominance. Clinical symptoms were polymorphic and nonspecific, but dominated by focal neurological deficits. Lesions were usually solitary and located at the supratentorial level. Pathological diagnosis was made after stereotactic biopsy in 32 patients and after surgical excision in 7 patients. The most common histological type was diffuse large B-cell lymphoma. All patients were included in the LCP protocol: polychemotherapy with high-dose methotrexate followed by consolidation with encephalic radiotherapy. Ten patients relapsed, including 8 in the brain and 2 in the testes. Overall, evaluation of the LCP protocol showed CR in 14 patients, death in 16 and 9 patients lost to follow-up. Overall and progression-free survival at 5 years was 40.36%, and 36.7% respectively.
