À ce jour, il existe plusieurs AMM pour un anti-IL1 chez des enfants même très jeunes. L’anakinra, dans certaines indications, a une AMM dès 8 mois et 10 kg ; après avis d’expert, il peut parfois même être initié chez des enfants encore plus jeunes et de plus faible poids. Plus l’enfant est de petit poids et plus le phénotype de la maladie est sévère, plus la dose à administre d’anti-IL-1 et notamment d’anakinra doit être élevée en mg/kg pour obtenir des résultats pharmacocinétiques et surtout d’efficacité satisfaisants. Chez ces enfants de petit poids comme chez les autres patients, il est important qu’il y ait mise à jour du carnet de vaccination. L’utilisation des anti-IL1 à des doses élevées peut potentiellement augmenter leur toxicité hématologique et hépatique ce qui incite à une surveillance plus rapprochée ces complications.

To date, there are several marketing authorisations for an IL-1 inhibitor in children, even very young. Anakinra, in certain indications, has a marketing authorisation from 8 months and 10 kg; after expert opinion, it can sometimes even be initiated in younger children and those with lower weights. The lower the child's weight and the more severe the phenotype of the disease, the higher the dose of IL-1 inhibitory, particularly anakinra, that must be administered in mg/kg to achieve satisfactory pharmacokinetic and, above all, efficacy results. In these low-weight children, as in other patients, it is important that their vaccination records are kept up to date. The use of IL-1 inhibitors at high doses may potentially increase their haematological and hepatic toxicity, which calls for closer monitoring of these complications.
