Le syndrome des antiphospholipides (SAPL) est une maladie auto-immune qui peut être primitive ou secondaire, le plus souvent associée au lupus érythémateux systémique (LES). Le SAPL est caractérisé cliniquement par des événements thrombotiques artériels ou veineux, des complications obstétricales et des anomalies biologiques associant la présence d’un anticoagulant circulant de type lupique et d’anticorps anticardiolipines et anti -β2 GPI . D’autres manifestations peuvent être observées, en particulier cardiaques. Il existe une forme sévère mettant en jeu de façon immédiate le pronostic vital, appelée syndrome catastrophique des antiphospholipides. Dans cet article, nous détaillerons les manifestations cliniques de ce syndrome, les critères de classification les plus récents ainsi que sa prise en charge, de la prévention primaire à la prise en charge en urgence du syndrome catastrophique.

Antiphospholipid syndrome (APS) is an autoimmune disease that may be primary or secondary, and is most commonly associated with systemic lupus erythematosus (SLE). APS is characterised clinically by arterial or venous thrombotic events, obstetric complications, and laboratory abnormalities comprising the presence of a lupus-type circulating anticoagulant and anticardiolipin and anti-β2-GPI antibodies. Other manifestations may be seen, particularly cardiac. A severe form exists that poses an immediate threat to life, known as catastrophic antiphospholipid syndrome. In this article, we detail the clinical manifestations of this syndrome, the most recent classification criteria, and its management, from primary prevention to the emergency management of catastrophic syndrome.
