Cluster of glue thistle (Atractylis gummifera L.) poisoning in children: a five-case outbreak disclosed by an index case in Tangier, Morocco

Introduction: Le chardon à glu ou Atractylis gummifera L. (Asteraceae) est une plante méditerranéenne dont les diterpènes glycosidiques (atractyloside et carboxyatractyloside) sont responsables d’une hépatonéphrotoxicité sévère. Au Maroc, il représente la première cause de décès par intoxication végétale, en particulier chez l’enfant. Nous rapportons un foyer d’intoxication collective accidentelle survenu en zone urbaine et révélé par un cas index pédiatrique.
Observations: Cinq enfants et adolescents (12 - 14 ans) d’un même quartier de Tanger ont consommé la racine de la plante au cours d’un jeu en plein air, autour du 30 mai 2026. Deux garçons (14 et 12 ans) ont développé une hépatite fulminante avec hypoglycémie profonde réfractaire, cytolyse hépatique majeure (ASAT à 19 144 et 10 550 UI/L ; ALAT à 13 122 et 9 313 UI/L chez les deux enfants), insuffisance rénale, coagulopathie et un œdème cérébral diffus chez le second. Ils sont décédés en moins de 24 à 48 heures malgré une prise en charge symptomatique et l’administration de N-acétylcystéine. La similitude clinique des deux premiers cas a conduit à la déclaration d’un foyer collectif et au déclenchement d’une recherche active, qui a permis d’identifier trois autres enfants exposés, restés asymptomatiques avec un bilan hépatique normal et une évolution favorable. La recherche urinaire de l’atractyloside par technique colorimétrique est revenue positive pour les deux cas mortels, aucune technique chromatographique de confirmation n’était disponible.
Conclusion: L’intoxication par Atractylis gummifera est redoutable chez l’enfant. Le pronostic est souvent aggravé par l’absence d’antidote et le délai de consultation. L’identification précoce du cas index et la déclaration au centre antipoison ont permis de repérer et de surveiller précocement les sujets co-exposés, illustrant l’intérêt de cette démarche comme outil de détection précoce et de toxicovigilance.

Introduction: Glue thistle ( Atractylis gummifera L.; Asteraceae) is a Mediterranean plant whose glycosidic diterpenes (atractyloside and carboxyatractyloside) cause severe hepato- and nephrotoxicity. In Morocco, it is the leading cause of death from plant poisoning, particularly in children. We report an accidental collective poisoning outbreak occurring in an urban area and disclosed by a pediatric index case.
Case reports: Five children and adolescents (12–14 years) from the same Tangier neighborhood ingested the plant root while playing outdoors, around May 30th, 2026. Two boys (14 and 12 years) developed fulminant hepatitis with refractory profound hypoglycemia, major cytolysis (AST 19,144 and 10,550 IU/L; ALT 13,122 and 9,313 IU/L), acute kidney injury, coagulopathy and cerebral oedema in the second case. Both died within 24 - 48 hours despite supportive care and N-acetylcysteine. The striking clinical similarity of the first two cases prompted outbreak notification and active case-finding, which identified three further exposed children who remained asymptomatic with normal liver tests and a favourable outcome. Urinary colorimetric screening for atractyloside was positive in both fatal cases, no confirmatory chromatographic technique was available.
Conclusion: A. gummifera poisoning is severe and often lethal particularly in children. The letality is often due to the lack of antidote and the delay before seeking medical care. Early recognition of an index case, poison-centre notification and active case-finding of co-exposed subjects are best regarded as tools for early detection and toxicovigilance rather than as a means of directly preventing fatal outcomes, which depend primarily on the severity of exposure.
