Introduction |
Le surpoids et l’obésité pédiatriques sont des problèmes de santé publique majeurs dont l’incidence est en constante augmentation. Réduire l’impact cardiovasculaire de l’obésité est un des objectifs de la prise en charge. La capacité aérobie de l’enfant obèse est diminuée et participerait à l’intolérance d’effort. L’étude de l’évolution du VO 2 max et des paramètres mesurés lors d’une épreuve d’effort cardiorespiratoire évalue les progrès de l’enfant et les éventuels bénéfices d’un programme de prise en charge.
Matériels et méthodes |
Le Programme de réhabilitation à l’effort dans le surpoids et l’obésité pédiatrique en Mayenne (RESOP 53) propose de l’activité physique adaptée avec une prise en charge pédiatrique, diététique et psychologique deux fois par semaine pendant 12 semaines aux enfants en situation d’obésité. Il s’agit d’une étude rétrospective.
Résultats |
Nous avons inclus 34 enfants (dont 53 % de garçon) avec une moyenne d’âge de 12,5 ans et un IMC moyen de 30,8 kg/m 2 (+2,99 DS) pris en charge dans le programme entre octobre 2021 et avril 2024. La différence de VO 2 max sur les épreuves d’effort entre le début et la fin du programme était de +150 mL/min ( p < 0,01) (bien supérieur aux 50 mL/min d’augmentation physiologique à cet âge), soit 7,2 % d’augmentation moyenne. Il y avait également une augmentation de la puissance maximale aérobie de 13 Watts, soit 11 % ( p < 0,01) et de la puissance au premier seuil ventilatoire (SV1) de 12 Watts, soit 18 % ( p < 0,01). Aucune différence n’était retrouvée sur le poids ou l’IMC.
Conclusion |
Notre étude met en évidence un gain immédiat du VO 2 max, de la puissance au maximum de l’effort et au SV1 en seulement 2 séances par semaine pendant 12 semaines. Ce gain est suffisamment important pour permettre un réel changement dans le quotidien de l’enfant souffrant d’obésité avec des mesures facilement transposable au quotidien. Une telle augmentation en si peu de temps suggère qu’il n’y aurait pas d’effet seuil de l’activité physique dans cette population de patients. Ces données sont à valider sur une étude prospective. L’absence de gain sur le poids ou l’IMC montre que l’efficacité d’un programme hospitalier sur une période relativement courte doit s’inscrire dans une prise en charge plus durable, principalement ambulatoire, pour permettre une modification des différents facteurs conduisant à l’obésité et permettre une perte de poids.

Introduction |
Pediatric overweight and obesity are major public health issues affecting an ever-growing number of children. Reduce the cardiovascular complications associated with obesity is a major goal in the management of the pathology. In obese children, aerobic fitness is diminished, which may contribute to exercise intolerance. The study of VO 2 max evolution and the parameters measured during a cardiorespiratory exercise test assesses the child's progress and the potential benefits of a fitness program.
Material and method |
RESOP 53 (Exercise Rehabilitation for Overweight and Pediatric Obesity in Mayenne) is a fitness program that consists of two weekly sessions of exercise tailored to each patient along with pediatric, dietary, and psychological care, for a total duration of 12 weeks. The program is intended for obese children living in Mayenne. It is a retrospective study.
Results |
Our study includes 34 children (including 53% of boys) who were enrolled in the program between October 2021 and April 2024. The children's average age is 12.5 years, and their average BMI is 30.8 kg/m 2 (+2.99 SD). The difference in VO 2 max during exercise tests between the beginning and the end of the program was +150 mL/min ( P < 0.01), which is significantly higher than the physiological increase of 50 mL/min at this age, representing a 7.2% average increase. There was also an increase in maximal aerobic power of 13 Watts, or 11% ( P < 0.01), and an increase in power at the first ventilatory threshold (VT1) of 12 Watts, or 18% ( P < 0.01). No difference was observed regarding weight or BMI.
Conclusion |
Our study reveals an immediate gain regarding VO 2 max, maximal power and power at the first ventilatory threshold with only two sessions per week over 12 weeks. This gain is sufficient to concretely improve the daily life of the child suffering from obesity. Such an increase over a short period suggests that there may not be a threshold effect of physical activity in this patient population. These data should be further validated with a prospective study. The absence of progress with respect to weight or BMI indicates that a short outpatient-based program such as RESOP 53 must be paired with long-term follow up of the patients to impact the multiple factors contributing to obesity and reach weight loss.
