18F-FDG PET/CT findings in an isolated polypoid nasal vestibular metastasis from locally advanced sarcomatoid renal cell carcinoma - 24/09/26

A 55-year-old man presented with a rapidly enlarging polypoid lesion in the right nasal vestibule associated with progressive nasal obstruction. Histopathological examination of the nasal vestibular biopsy demonstrated metastatic renal cell carcinoma with sarcomatoid differentiation. Whole-body 18 F-FDG PET/CT was performed for initial staging following intravenous administration of 8 mCi (296 MBq) of 18 F-FDG. The blood glucose level before tracer injection was 110 mg/dL, and image acquisition was performed 60 mins after injection using an Ingenuity TF 64 PET/CT scanner (Philips Medical Systems, Cleveland, OH, USA). PET/CT demonstrated intense FDG uptake in the right nasal vestibular lesion (SUVmax 5.5) and a large hypermetabolic right renal mass (SUVmax 5.6) with direct invasion of the adjacent duodenum. No additional metastatic lesions were identified. Histopathological examination of the nephrectomy specimen confirmed sarcomatoid renal cell carcinoma. This case highlights the complementary role of 18 F-FDG PET/CT in identifying an occult primary renal malignancy, accurately defining disease extent, confirming the nasal vestibular lesion as the only distant metastatic site, and facilitating appropriate surgical management.

Un homme de 55 ans a consulté pour une masse polypoïde rapidement évolutive du vestibule nasal droit responsable d’une obstruction nasale progressive. L’examen histopathologique de la biopsie a révélé une métastase d’un carcinome rénal à différenciation sarcomatoïde. Une TEP/TDM au 18 F-FDG réalisée pour le bilan d’extension après injection intraveineuse de 8 mCi (296 MBq) de 18 F-FDG a montré une glycémie pré-injection de 110 mg/dL. Les images ont été acquises 60 min après l’injection à l’aide d’un système Ingenuity TF 64 PET/TDM (Philips Medical Systems, Cleveland, OH, États-Unis). L’examen a mis en évidence une hyperfixation intense de la lésion du vestibule nasal droit (SUVmax 5,5) ainsi qu’une volumineuse masse hypermétabolique du rein droit (SUVmax 5,6) avec invasion directe du duodénum adjacent. Aucun autre foyer métastatique n’a été identifié. L’examen anatomopathologique de la pièce de néphrectomie a confirmé un carcinome rénal sarcomatoïde. Ce cas souligne le rôle complémentaire de la TEP/TDM au 18 F-FDG dans l’identification d’une tumeur rénale primitive occulte, l’évaluation précise de l’extension tumorale et la planification thérapeutique.
