Résumé |
Face aux limites du modèle anatomoclinique dans la compréhension des rachialgies communes, cet article montre que la souffrance au travail a un impact majeur dans le déclenchement et la chronicisation des rachialgies. En appui sur la psychodynamique du travail, l’article qui suit illustre, vignette clinique à l’appui, comment des dynamiques managériales délétères peuvent fragiliser le sujet jusqu’à l’effondrement psychosomatique. En réponse à ces constatations, une approche multidisciplinaire combinant la rhumatologie, la médecine du travail et la psychologie clinique permet de rendre le patient à sa subjectivité.

As the anatomo-clinical model shows its limitations to explain chronic back pain, this article highlights the major impact of suffering at work on back pain and its chronical evolution. Using the framework of the psychodynamics at work, this article shows with the help of a clinical follow up, how deleterious managerial dynamics can lead an employee to a psychosomatic decompensation. These constatations demonstrate that a multidisciplinary approach (combining rheumatology, occupational medicine, and psychology) enable to restore the patient's subjectivity.
