Background |
Trichotillomania is a body-focused repetitive behavior disorder that may remain hidden for years. In rare cases, trichophagia can occur potentially leading to severe gastrointestinal complications, such as the formation of trichobezoars.
Case presentation |
We describe a 14-year-old girl who developed a gastric perforation caused by a trichobezoar, requiring emergency surgery. Psychiatric evaluation revealed a longstanding history of automatic hair-pulling and ingestion, associated with high anxiety and emotional inhibition. Integrated treatment combining psychoeducation, family work, cognitive-behavioral strategies, and pharmacotherapy led to cessation of trichophagia, significant reduction in hair-pulling, and improvement in anxiety symptoms.
Conclusions |
This case highlights the mind-body connections underlying body-focused repetitive behaviors and the importance of multidisciplinary collaboration. It illustrates the intersection between physical risk and emotional-regulation difficulties, emphasizing the value of a person-centered, formulation-based approach in adolescent psychiatry. Early recognition and integrative care are essential to prevent medical complications and improve long-term outcomes.

Introduction |
La trichotillomanie est un trouble du comportement répétitif centré sur le corps qui peut rester dissimulé pendant plusieurs années. Dans de rares cas, une trichophagie peut survenir, pouvant entraîner de graves complications gastro-intestinales, telles que la formation de trichobézoards.
Observation |
Nous décrivons le cas d’une adolescente de 14 ans ayant présenté une perforation gastrique causée par un trichobézoard, nécessitant une chirurgie en urgence. L’évaluation psychiatrique a mis en évidence une histoire ancienne de tirage et d’ingestion de cheveux à caractère automatique, associée à une anxiété élevée et à une inhibition émotionnelle. Une prise en charge intégrée combinant psychoéducation, travail familial, stratégies cognitivo-comportementales et traitement pharmacologique a permis l’arrêt complet de la trichophagie, une réduction significative du tirage de cheveux et une amélioration nette des symptômes anxieux.
Discussion et conclusion |
Ce cas met en évidence les liens entre le corps et l’esprit dans les comportements répétitifs centrés sur le corps et souligne l’importance d’une collaboration multidisciplinaire. Il illustre l’interaction entre le risque somatique et les difficultés de régulation émotionnelle, mettant en avant la valeur d’une approche centrée sur la personne et fondée sur la formulation en psychiatrie de l’adolescent. Une reconnaissance précoce et une prise en charge intégrative sont essentielles pour prévenir les complications médicales et améliorer le pronostic à long terme.
