Association of bone density, cortical bone thickness, and planned intrabony depth with the accuracy of guided palatal miniscrew insertion in adult patients: A prospective clinical study - 29/10/26
, Ahmed Mohamed Kamel, Marwa Ali TawfikSummary |
Objective |
The primary objective of this study was to evaluate the association between bone density (BD) as the primary predictor and global tip deviation, representing the primary outcome measure of guided miniscrew insertion accuracy. Secondary objectives were to explore the associations of BD with global head deviation and angular discrepancy, and of cortical bone thickness (CBT) and planned intrabony insertion depth with all deviation measures.
Material and methods |
This prospective clinical study included 46 self-drilling palatal miniscrews inserted in 23 patients requiring intrusion of maxillary posterior teeth in open bite patients. Virtual miniscrew placement was planned using CBCT and IOS. BD, CBT, and planned intrabony insertion depth were assessed at the insertion sites. Postoperative IOS were obtained using scan bodies attached to the inserted miniscrews, planned and postoperative digital models were superimposed for three-dimensional deviation analysis. Linear deviations were measured at the miniscrew head and tip along the transverse (X), sagittal (Y), and vertical (Z) axes, with global three-dimensional deviations also calculated. Three-dimensional angular discrepancy of miniscrew axes was assessed.
Results |
BD demonstrated significant positive correlations with tip global deviation (r = 0.379, P = 0.009), tip vertical deviation (r = 0.302, P = 0.041), and angular discrepancy (r = 0.403, P = 0.005). CBT showed no significant association with any deviation measurements. The mean global deviations were 0.941 ± 0.385 mm and 3.208 ± 1.86 mm at the miniscrew head and tip, respectively. The mean angular discrepancy was 8.585 ± 3.950°. No significant differences were observed between right- and left-side miniscrews ( P > 0.05). No harm was observed during the trial.
Conclusion |
Guided palatal miniscrew insertion produced variable transfer deviations, with mean deviations of 0.941 ± 0.385 at the screw head and 3.208 ± 1.863 at the tip. Increased BD was associated with greater tip and angular deviations.
