Le diagnostic d'hépatite C est évoqué dans trois circonstances : une symptomatologie évocatrice, la découverte d'une élévation de l'activité sérique des aminotransférases, ou la découverte d'une sérologie C positive lors d'un bilan systématique. Le diagnostic virologique de l'hépatite chronique C repose sur la mise en évidence des anticorps anti-virus de l'hépatite C (VHC) par test immuno-enzymatique. La recherche d'ARN viral doit être limitée aux circonstances où la démonstration d'une réplication du virus est nécessaire, ainsi qu'à l'étude de la réponse aux traitements anti-viraux. La mesure de la charge virale et la détermination du génotype avant traitement devraient être utilisées dans l'avenir pour déterminer la durée optimale du traitement par l'association interféron alpha-ribavirine.

The diagnosis of hepatitis C can be evoked in three settings : symptoms of liver disease, elevated serum alanine aminotransferase activity, and positive anti-HCV detection on systematic screening. The diagnosis is based on anti-hepatitis C virus (HCV) antibody detection by means of enzyme immuno-assays. HCV RNA detection should be restricted to the settings in which knowing the replicative status of the virus is needed, and to the assessment of the virological response to antiviral therapy. Pretreatment determination of viral load and HCV genotype should be used in future to tailor interferon plus ribavirin combination therapy duration.
