Objectifs. - Objectif principal : évaluer l'impact de la diffusion de recommandations en antibiothérapie curative sur le taux de prescriptions adaptées à l'indication au centre hospitalier de Lagny-Marne-la-Vallée (CHLMV). Objectifs secondaires : évaluer l'impact de la diffusion de recommandations en antibiothérapie curative sur le taux d'ordonnances adaptées à l'ensemble des critères d'adéquation définis, sur l'écologie bactérienne et les dépenses d'antibiotiques. Estimer le taux d'infections nosocomiales.

Matériel et méthodes. - Comparaison des relevés des ordonnances détaillées, effectuées sur une période de trois mois avant et un an après diffusion d'un livret de recommandations en antibiothérapie curative. Les relevés sont effectués dans les mêmes conditions, de manière prospective sur trois mois et analysés par critère d'adaptation. Les critères utilisés sont ceux déterminés pour l'étude effectuée avant la rédaction des recommandations.

Résultats. - Augmentation significative du taux d'ordonnances adaptées à l'ensemble des critères d'adéquation mais aucun impact sur le taux d'adaptation à l'indication. Diminution nette des prescriptions inadéquates pour motif économique. Augmentation du taux d'ordonnances inadaptées pour association non prescrite.

Objectives. - Main objective: the authors wanted to evaluate the impact of recommendations for curative antibiotic therapy on the rate of prescriptions adapted to the indication, in the Lagny-Marne-la-Vallée Hospital. Secondary objectives: they also wanted to evaluate: the impact of recommendations for curative antibiotic therapy on the rate of prescriptions adapted to all of the defined suitability criteria, on bacterial ecology, and on the cost of antibiotics ; and the rate of nosocomial infections.

Materials et methods. - Detailed prescriptions were compared over 3 month period, before and one year after distribution of recommendations for curative antibiotic therapy. The prescriptions were made under the same conditions, prospectively over 3 months, and analyzed using each suitable criterion. The criteria used were those designed for the study which was made before recommendations were edited.

Results. - A significant increase was noted in the number of prescriptions adapted to all of the suitability criteria, but there was no impact on the rate of suitability for the indication. There was a marked reduction in unsuitability due to economic reasons, and an increase in the rate of unsuitable prescriptions due to a non-prescription of association of antibiotics.
