Résumé |
Introduction |
Les manifestations neurologiques centrales du syndrome de Gougerot-Sjögren primitif (SGSP) sont très polymorphes, parmi lesquelles les pachyméningites représentent l’une des manifestations les plus rares, elles sont exceptionnellement révélatrices.
Cas clinique |
Une patiente âgée de 50 ans admise pour syndrome d’hypertension intracrânienne avec paralysie du III et du VII. L’IRM encéphalique avait objectivé un aspect de pachyméningite en regard de l’hémisphère cérébral droit. Le bilan étiologique était négatif. L’interrogatoire a révélé l’existence d’une xérostomie et d’une xérophtalmie. La biopsie des glandes salivaires accessoires a objectivé une sialadénite stade III de Chisholm, la recherche d’anti-SSA et d’anti-SSB était positive. Nous avons conclu à un SGSP. L’évolution sous corticothérapie et cyclophosphamide a permis la régression de l’atteinte neurologique.
Conclusion |
À travers cette observation, nous soulignons la rareté des pachyméningites au cours du SGS primitif et leur caractère exceptionnellement révélateur.

Introduction |
Central neurological manifestations of primary Sjogren’s syndrome are very polymorphic, including pachymeningitis, one of the rarest manifestations, which is exceptionally inaugural.
Clinical case |
A 50-year-old patient was admitted for an intracranial hypertension syndrome with paralysis of the III and VII cranial nerves. Brain MRI revealed pachymeningitis of the right cerebral hemisphere. History taking revealed the existence of xerostomy and xerophthalmos. The accessory salivary gland biopsy demonstrated Chisholm stage III sialadenitis. Search for anti-SSA antibodies and anti-SSB antibodies was positive. The diagnosis of primary Sjogren’s syndrome was retained. The patient improved with corticosteroid therapy and cyclophosphamide.
Conclusion |
This case illustrates the unusual observation of pachymeningitis in primary Sjogren’s syndrome in the rare setting as an inaugural manifestation.
