Progression of a prepyloric hyperplastic polyp - 23/08/11
| Commentary The vast majority of benign gastric polyps not related to proton pump inhibitor use are hyperplastic. Most of them are likely to remain relatively unchanged, although we are increasingly being made aware of adenomatous change with development of carcinoma in some. Here we have another noteworthy development, namely increase in size with subsequent prolapse into the duodenum and gastric outlet obstruction. Such progression has been reported previously but is not predictable, and, absent a germline mutation that increases the risk for malignancy, certainly doesn’t change the general perception that small (hyperplastic) gastric polyps should be observed rather than removed to prevent carcinoma or, as in this case, obstruction.
