Accrual of bone mass during childhood is a major determinant of osteoporosis risk in adulthood. This article reviews expected gains in bone mass and modifiable determinants of bone mineralization during growth. The strengths and weaknesses of available methods to assess bone health are discussed, as are analytic approaches to the interpretation of bone density measures. Finally, two disorders, inflammatory bowel disease and cystic fibrosis, provide examples of disrupted bone mineralization in childhood disease.
